bill all charges for services electronically with your insurance
company, and their payment will be listed on your statement.
Please inform the nurse when you first arrive of any health
insurance coverage you have. This can be a complex process,
and we ask that you help us to prevent complications by the
following:
n Bring your current health
insurance card to each visit. We must have a copy of your
card to bill your insurance carrier.
n Pay any co-payment that is due at the time of your
visit.
n Let us know when you arrive if your insurance has
changed or if you are temporarily without insurance.
n Call our billing office at 572-3145, or e-mail
us, if you are having trouble paying your bill.
n If you have problems or questions about a bill from
an outside diagnostic lab, please call the medical office
at 572-3140. While Family Health Care is not responsible for
their charges, our nurses or doctors can often help in sorting
them out. |

You are Uninsured
Almost everyone has had times when they are sick and have
no insurance for paying the costs of health care. This occasionally
occurs with our current patients, and we have a long tradition
of arranging discounts on our usual full charges to help them
with their medical expenses.
Also, many people are forced to purchase insurance that requires
payment for many services before insurance applies. Deductibles
can be as much as $5000. We are always willing to discuss
options for a reasonable adjustment in our fees when a person
has a high-deductible plan.
For new patients who are uninsured, we have a limited capacity
to offer discounts. If you are in this situation and wish
to schedule a “first appointment," we require a
cash payment of $50. to be made to the nurse before you are
seen. During the visit, the doctor will discuss a discount
plan for future appointments.
